1. Change fatigue - so many changes, so many initiatives. They come from all directions and at different angles. There are rarely clear benefits or reason. I wonder how many failed [perceived unnecessary] initiatives it takes to damage the chances of the really important one, no matter how well planned or positioned.
2. Creeping Complacency - I can almost hear "We've been working on this for years now. The time and money investment has been huge, surely there isn't anything else we need to do? We haven't seen a failure for quite some time and even the last audit said we were in good shape".
